Elementary Schools

Doctor's orders are very important when your child gets injured. It gives specific instructions about your child's injury and how we can best care for it so they can recover as quickly as possible.  

• If your child needs to be excused from gym, please have a DOCTOR'S ORDER brought in or faxed to the HEALTH OFFICE stating they can not participate in gym AND for how long.  Have them write an order stating any other restrictions that we need to know about. 
• In order for them to return to gym, please have a DOCTOR'S ORDER brought in or faxed stating the date they are able to return to gym.
